    $43/hr


    Schedule:
    Overnights (NOC) - Full-time

    Shift: 11:00 pm - 7:00 am, with an every other weekend rotation

    Sign On Bonus:
    $2,500 sign on bonus for one year commitment payable 50% after 90 days with remainder paid after 180 days of full time employment

    A little more about our center:


    The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit.

    This close-knit team is welcoming and always willing to jump in to help out when you need it.


    What makes us unique:
    Highly ranked Quality Measures – this means we provide excellent clinical care
    30% of our residents are late stage memory care patients


    NOTE:
    we are one of the only nursing homes in the area with specialized dementia programming.

    We have a busy Activities Department that keeps our residents engaged with low level sensory rooms, cognitive therapeutic games, regular entertainment and cooking classes.

    There is a bus stop located in front of the center and many apartment homes nearby if you want to move closer to work.


    The National Association of Rehab Providers & Agencies (NARA) awarded Transitional Care Management their "Impact Award" for 2023 for our Life Skills Programming.

    What you will do in this role:


    As a Registered Nurse (RN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care.

    Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

    Our Floor Nurses are among the best & they would love to have you join them.

    Qualifications


    What we need from you:
    Registered Nurse (RN) licensed in IL
    Prior experience in a SNF or ICF preferred
    Newly licensed Nurses are welcome to apply
    Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

    This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area.

    These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
