- Provides a full range of nursing care to patients with complex care needs, assuming responsibility and accountability for nursing actions.
- Completes initial assessments and reassessments of patient condition and response to treatment and other clinical findings. Initiates follow-up as needed.
- Assesses, develops and implements a plan to meet patient's learning needs, including self-care and treatment measures.
- Modifies approach based on knowledge of human behavior, motivations, and reactions to situations.
- Frequently engages co-workers/team in active problem-solving regarding clinical issues leading to an improvement in care.
- Serves as a resource to other staff in meeting individualized learning and discharge needs, recommends appropriate referrals, adaptive equipment and approved educational materials.
