Overview:
We are seeking a dedicated and compassionate Physical Therapist to join our home health team.

As a Physical Therapist, you will play a vital role in providing quality physical therapy services to our clients in the comfort of their homes.

This position offers
per diem pay and is an excellent opportunity for experienced professionals or recent graduates looking to make a positive impact in the field of physical therapy.


Duties:


  • Evaluate clients' conditions and develop individualized treatment plans based on physician prescriptions and assessments.
  • Provide handson physical therapy services to clients, including therapeutic exercises, manual therapy techniques, and modalities.
  • Monitor clients' progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Educate clients and their families on proper techniques, exercises, and home care strategies to promote recovery and independence.
  • Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to ensure coordinated care and comprehensive treatment approaches.
  • Document all aspects of client care accurately and timely in accordance with agency policies and regulatory requirements.
  • Participate in staff meetings, inservice trainings, and professional development activities to enhance clinical skills and knowledge.

Qualifications:

  • Graduate of a physical therapy program approved by the American Physical Therapy Association or equivalent accrediting body.
  • Licensed or registered by the state of California as a Physical Therapist.
  • Preferably, at least one year of experience in a clinical or home health setting.
  • Strong clinical assessment and treatment skills, with knowledge of various physical therapy techniques and modalities.
  • Excellent communication and interpersonal skills to effectively interact with clients, families, and healthcare team members.
  • Ability to work independently and collaboratively in a dynamic healthcare environment.
  • Must have a car with required insurance coverage and a valid State driver's license for home visits.

Responsibilities:


  • Conduct comprehensive evaluations of clients' physical and functional abilities to develop individualized treatment plans.
  • Provide direct physical therapy services to clients according to established treatment goals and plans of care.
  • Educate clients and their families on proper techniques for exercises, mobility aids, and home safety measures.
  • Document all client interactions, assessments, interventions, and progress notes in accordance with agency policies and regulatory standards.
  • Collaborate with interdisciplinary team members to ensure coordinated and holistic care for clients.
  • Participate in ongoing quality improvement initiatives and performance improvement activities.
  • Maintain professional competency through participation in continuing education programs and professional development opportunities.

Functional Abilities:

  • Must be able to read and comprehend client records and documentation.
  • Must have clear verbal communication skills to interact effectively with clients and healthcare team members.
  • Ability to perform physical tasks such as stooping, bending, lifting, and carrying during client evaluations and treatments.
  • Must be able to travel to clients' residences within the designated service area.
