Director of Clinical Operations, Nurse Practitioner (NP)

Job Summary:

The Director of Clinical Operations (DCO) will be responsible for the oversight and planning of the clinical functions and staff of the Skilled Nursing Facility (SNF) clinical division of Personalized Health Partners.

This will include oversight of providers in partnering SNF facilities.

The DCO will work in conjunction with the VP of Clinical Operations to identify and implement new business, clinical processes, and monitor clinical goals.

The DCO will be a leader in the organization's strategic clinical, quality, and financial goals.

PRIMARY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:


  • Oversee the daily operations of clinical team
  • Promote ongoing team education in clinical as well as nonclinical responsibilities
  • Serve as the primary clinical liaison for all clients and key stakeholders
  • Lead, plan, and execute clinical implementation strategies and initiatives in partnership with the operations team for all new clients
  • Serve as the liaison between the SNF practice and telehealth software on clinical outcomes, goals, and concerns
  • Provide ongoing performance feedback throughout the year, counsel employees in disciplinary matters, and obtain assistance from Human Resources for disciplinary actions and/or termination.
  • Ensure compliance with regulatory guidelines and system policies/procedures for all activities within the practice
  • Identify opportunities for process and operational improvement
  • Identify/anticipate education opportunities for clinical team
  • Develop relationships with key stakeholders within the SNF's, Corporate Leadership and other external partners
  • Follow all state and federal regulations, guidelines, and laws
  • Other duties as assigned.

EDUCATION and QUALIFICATION:


Required:


  • 5+ years of clinical experience
  • 5+ years in a clinical or operations management role
  • Active, unrestricted state license (NP)
  • US work authorization
  • 2+ years clinical experience as a Nurse Practitioner in geriatric, adult or family practice setting
  • Ability to work in a flexible environment and schedule
  • Passionate about providing healthcare to the senior and lowincome populations

Preferred Qualifications:


  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification of AANP or ANCC
  • Active DEA license
  • Experience with coding, ICD10 and HCC (Hierarchical Condition Categories)
  • Experience or training working in a metrics, results driven healthcare model
  • Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
  • Experience working in valuebased care models
  • Training or experience in geriatric, assisted living, and/or skilled nursing setting
  • Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
  • Training or experience in outpatient primary care setting

JOB SKILLS:


  • Must have strong oral and written communication skills
  • Must be detail oriented with an ability to work well both independently and in a team setting
  • Exhibit a customer service approach with teams and patients
  • Strong time management skills required. Must be able to prioritize and adhere to competing deadlines while achieving goals
  • Must be willing to travel when needed (50%).
