Job Summary:
Hartwood Foundation is seeking a dedicated Residential Service Coordinator to ensure the overall wellbeing of our service recipients.
To act as a liaison to medical practitioners, pharmacies and suppliers; provide support/assistance to service recipients and staff to ensure appropriate coordination and delivery of medical/healthcare services; oversee efforts to comply with regulatory and contractual obligations associated with the healthcare support of service recipients:
coordinate agency efforts relevant to enviornment safety. The Residential Coordinator will be responsible for managing programs, supervising staff, and ensuring the well-being of residents.
Duties:
- Assure routine assessment of service of service recipient health/medical support needs in cooperation with residential service staff.
- Review physicians' orders and other relevant documentation to assure accurate completion and implementation as appropriate.
- Monitor medical appointments for appropriate scheduling and followup activities as necessary and in cooperation with team members.
- Assure service recipient medication is evaluated at least quarterly by a licensed physician.
- Provide routine reports and recommendation for immediate and longterm consideration based on observations and documentation of activities related to service recipient health.
- Coordinate and monitor an education component that will identify and correct problems to ensure service recipient health, safety and general wellbeing.
- Ensure accurate maintenance of service recipient health and medical needs.
- Identify training needs specific to appropriate medical and health service coordination
- Analyse and implement new regulations where applicable.
Skills:
- Master's degree in related field and
- Strong program management skills years related experience, one year being supervisory experience OR Bachelor's degree and three years related experience, one year being supervisory.
- Excellent command of English language usage both oral and written, basic mathematics concepts, basic concepts of personnel management.
- Experience in teaching of healthcare practices preferred.
- Ability to function independently to include goals and objectives and responding to verbal and written instructions and guidelines.
- Knowledge of behavioral health practices
- Ability to establish and maintain effective working relations with service recipients, staff, families, community representatives and other professionals.
- Experience working with individuals with intellectual and developmental disabilities.
Pay:
$74, $76,000.00 per year
