THE POSITION:


This is a position for a caring and responsible person committed to providing families and individuals the highest quality of behavioral health care services.

We are seeking a patient person with good judgment, organizational skills and the ability to communicate with all ages. We offer a great supportive team atmosphere and an excellent benefit package. Training is provided.


DESCRIPTION OF WORK:

Develops plans for Intellectual Disability consumers following intake process. Develops treatment plans and enrollment into Behavioral Health/Intellectual Disabilities/Early Intervention/Autism programs as appropriate. Develops Individual Support Plans (ISP) with detailed goals. Locates providers as detailed on the ISP.

Maintains files, records and other documentation for the continuity of care in a timely and organized manner. Develops life management plans and acts as an advocate for the consumer to ensure appropriate treatment as approved. Documents contacts with consumer, family, and service providers.

Duties are not limited to desk work and may include frequent standing, climbing steps and walking distances. Employee must be able to climb flights of steps in homes or apartment buildings where no elevators are present. May work more than eight hours per day. Position involves frequent driving both in and out of County, with some out of county, overnight travel. Works 37.5 hours (full-time) per week, generally 8am to 4:30pm with an hour for lunch.

Manages multiple tasks, paying close attention to details under a constantly changing work environment which includes completion of paperwork and meeting strict deadlines.

Completes daily deadlines on paperwork and work assignments.


Work Schedule and Additional Information:


  • Fulltime employment.
  • Work hours are 8:00 am to 4:30 pm, Monday
  • Friday, with a 1hour lunch.
  • Occasional overnight and out of County travel

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY:


QUALIFICATIONS

Minimum Experience and Training Requirements:


  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
    or:
  • A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
    or:
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Other Requirements:


  • PA residency requirement is currently
    waived for this title.
  • You must be able to perform essential job functions.

Legal Requirements:


  • A conditional offer of employment will require a drug screening.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
