Registered Nurse - RN - Home Health

Optimal Home Care is looking for a Registered Nurse to provide services to our patients.

The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes.

When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.

A graduate of an accredited, approved school of professional nursing

Must be able to communicate in the language of the client and document client related activity as appropriate.
At least one (1) year of experience as a Registered Nurse in Home Health.
Due to the requirements mandated by the Colorado Board of Health, you must be fully vaccinated to be considered for this position with Optimal Home Care, Inc.
