- As the primary BOLD liaison for the Alzheimer's Association in Virginia, provide oversight and leadership within the Chapter, with the Department of Health and with BOLD Partners to execute on the grant strategies
- Represent Virginia Chapters by serving on the Virginia Department of Health BOLD jurisdiction coalition
- Provide information and education to BOLD coalition members/partners on ADRD related topics
- Provide education and awareness throughout Virginia to the general public on ADRD topics with a special emphasis on populations of high burden
- Complete all necessary reporting and evaluation requirements as defined in the subaward
- Work with key partners to expand reach, membership and impact (taking leadership roles where possible) to develop and implement goals consistent with the BOLD Scope of Work
- Build relationships with diverse partners to promote health equity and ensure inclusion for Virginians from all backgrounds and communities
- Lead planning to host annual conference with partners on brain health among diverse communities
- Work in partnership with Association staff throughout the state to plan and host Community Forums in underserved and underrepresented communities
- Disseminate information around current data, interventions, best practices to support brain health, address cognitive impairment, improve early detection and diagnosis, and support caregivers
- Manage EssentiALZ recruitment and enrollment of long-term care providers throughout the state with an emphasis on rural and underserved communities
- Manage the recruitment of primary care practitioners to the Alzheimer's Association Project ECHO program
- Disseminate the Dementia Care Practice Recommendations, Guide to Quality Care from People Living with Dementia, and utilize Alzheimer's Association tools to refer patients to local chapters to access supportive programs
- Proven success in building and maintaining collaborative relationships and meeting strategic goals through partnership initiatives.
- Able to identify and cultivate relationships with community partners, organizations, volunteers and diverse populations to create a sense of urgency related to dementia awareness as a major health issue and motivate all to action.
- Ability to effectively collaborate with others, hold others accountable and reach mutually agreeable outcomes.
- Familiarity with a public health approach focused on health promotion and early detection.
- Experience building rapport/trust, assessing needs and articulating a value proposition.
- Ability to represent the Association effectively to community partners, community leaders and healthcare partners.
- Excellent written and presentation skills, comfortable speaking in public
- General knowledge of public and community health, community-based organizations and systems
- Familiarity working on grant funded project preferred
- Bachelor's degree in related field or equivalent experience
- Education/expertise in Public Health
- Three years of experience in program management/delivery and evaluation
- Awareness/understanding of dementia as a public health issue and the role of early intervention in delaying cognitive decline
- Familiarity with the diverse communities that make up the State of Virginia
- Ability and willingness to travel up to 60% within the assigned territory by car or air

POSITION SUMMARY:
The purpose of this role is to lead the implementation of strategies identified in the Alzheimer's Association subaward to the Virginia Department of Health's BOLD action plan and to represent the Alzheimer's Association by collaborating with statewide partners to advance Virginia's dementia infrastructure through a public health approach to brain health and health equity. The Manager is responsible for delivering brain health education programs, educating healthcare providers and working closely with the Virginia Department of Health and BOLD Coalition partners to ensure the effectiveness of the statewide coalition to ensure successful completion and timely reporting related to the sub award and implementation of priority activities identified under the Healthy Brain Initiative (HBI) Road Map.

Funding for this position is made available through CDC's BOLD Public Health Programs to Address Alzheimer's Disease and Related Dementias (CDC-RFA-DP initiative. Grant-funded positions are time-limited and contingent upon sufficient funding. While the anticipated project period is 5 years, CDC will continue to award funding annually based on the availability of funds.
