Position Summary:
The Youth Housing Case Manager works to identify the needs of homeless youth (ages 18-24), connects them to resources, and helps them form a plan for obtaining long-term housing and financial stability.
This role will take leadership responsibility in coordinating and providing case management services for clients enrolled in the Youth Housing Program.
Responsibilities:
- Provide Case Management Services to households and/or single individuals in the Joint Youth Transitional/Rapid Re-housing Program in Waukesha, Ozaukee, and Washington Counties.
- Coordinate and provide care that is safe, effective, trauma-informed, and client-centered.
- Maintain accuracy and organization in client files.
- Meet with clients to assess their needs and form individualized plans for housing stability, employability, and long-term financial wellbeing.
- Oversee life-skills coordination, which develops clients' ability to meet daily responsibilities and improve their potential to live independently.
- Work with community partners to refer clients to mental health assistance, job skills training, resume assistance, and other services.
- Enter guests into the Coordinated Entry system in a timely manner; maintain accurate records on the Wisconsin Clarity database, including Coordinated Entry, case notes, and referrals.
- Monitor and adjust client statuses based on changing needs and conditions.
- Help clients make informed decisions by acting as their advocate regarding their current situation.
- Work in conjunction with the Housing Program Manager to help complete tasks and achieve goals.
- Collaborate with the leadership team, shelter staff, and case management team.
- Participate in regular meetings with program/shelter staff.
- Flexibility to work hours outside of regularly scheduled time.
- Participate in regular ongoing training.
- Perform other duties as assigned by the Housing Program Manager
Skills/ Qualifications:
- Bachelor's degree preferred.
- Minimum 2year degree in Social Work, Human Services, or another related field.
- Knowledge of Trauma-Informed care
- Strong organizational skills and ability to work independently.
- Ability to communicate effectively, including verbal, written, and active listening skills.
- Ability to establish and maintain personal and professional boundaries in working with clients.
- Ability to maintain a quality workplace in a diverse, fastpaced, stressful, and changing environment.
- Access to a reliable vehicle for meeting with clients in the community
- Ability to assess emergency situations and respond effectively.
- Follows through with commitments and meets deadlines.
- Demonstrates a strong ability to perform tasks with mínimal supervision or oversight.
- Demonstrates ability to be an effective team member.
Schedule:
This hourly, non-exempt position requires a 40-hour work week and has first shift availability. Some weekends and evenings may be required.
Pay:
$ $24.00 per hour
