    JOB PURPOSE:
    The information below covers the role requirements, expected candidate experience, and accompanying qualifications.


    The Nurse Practitioner provides primary care to patients in the Clinic setting under a collaborative relationship with the Medical Director.

    Practices within his/her scope of practice as delineated by his/her education, training and clinical protocols

    ESSENTIAL FUNCTIONS INCLUDE BUT NOT LIMITED TO:
    Under direction of physician or other medical personnel, administers medication to patients.
    Collaborates with other healthcare professionals to provide efficient quality patient care.
    Evaluates and improves teaching and nursing practices and recommends modifications.
    Completes histories, progress notes and treatment recommendations in patient medical record.
    Provides services in accordance to policy.
    Assumes lead role to ensure policies, procedures, and physician's orders are followed.
    Arranges for or refers patients to needed services that cannot be provided at the clinic.
    Conducts laboratory, x-ray and specialty referral of clinic patients.
    Charting receives acceptable standards during quality reviews.
    Writes prescriptions following the arrangements stipulated within the collaborative practice agreements and clinical protocols that adhere to state guidelines.
    Answers routine questions from patients, family, and care givers.

    Supports policies and procedures and supervisory decisions within the clinic and participates in the development, execution, and periodic review of patient health records within a collaborative relationship.

    Actively and consistently contributes to the clinic operations.
    Accepts supervision and feedback from the Medical Director.
    Promotes effective and cooperative relationships with referral providers.
    Understands and implements patient rights.
    Continues education through in-service training, seminars, and literature in order to maintain up-to-date knowledge of current trends in healthcare.
    Performs various other duties as needed to successfully fulfill the function of the position within scope of practice.

    Adhere to the Avem Health Partners Code of Conduct and Standards of Behavior. Complies with established policies and procedures and all health and safety requirements.


    EDUCATION/QUALIFICATIONS:
    Master's Degree in Nursing
    Strong interpersonal communication skills.
    Experience in healthcare services for geriatric patients in rural health clinics and skilled nursing facilities preferred.


    CERTIFICATION/LICENSURE:
    Current Oklahoma RN license and certified as a Nurse Practitioner by the Oklahoma Board of Nursing.
    Current BLS, ACLS and PALS (or obtained within 30 days of hire)
