    Position Summary & Responsibilities

    Position Summary:
    This position is essential in coordinating and performing processes related to treatment planning and patient information functions.

    Specific areas include timeliness, providing summary of significant occurrences for review period, checking orders/reports and communicating with physician/APRN for medical diagnoses that need to be added to plan, and assurances of interdisciplinary contributions to the treatment planning process in addition to aiding in the adequacy and timeliness in the provision of treatment goals, objectives, modalities and interventions, to meet the needs of the patient, treatment team and rules and regulations of certifying and accrediting agencies, accurate tracking of Tier level in Plan and accurately recording polygraph in the plan.

    This position is essential to preparation, maintenance and completion of patient records.

    This position will also use job knowledge to recommend implementation of the desired quality changes designed to support change to programs and support the agency mission and vision.

    Job Responsibilities may include but are not limited to the following:

    Treatment Process Coordinator Responsibilities


    • Schedules Comprehensive Integrated Treatment Plan (CITP) in accordance with timelines.
    • Notifies and provides Treatment Team with schedule of CITPs to be reviewed.
    • Provides accurate account of significant occurrences during review period to therapist
    • Coordinates with nursing staff in reviewing significant occurrences for inclusion of new goals in Treatment Plan.
    • Checks with medical staff for updates on physical diagnoses
    • Compiles and types CITPs
    • Reviews Falls Assessment (CPR-84) for inclusion in CITP prior to staffing
    • Enters resident demographics prior to treatment team meeting
    • Ensures Probation requirements are clearly set out in CITP as needed
    • Reviews /Updates:
    • Problems/LTGs/STGs
    • Diagnosis according to clinical staff input
    • Updates Progression towards Long Term Goal under Sexual Deviancy Problem by checking of latest Polygraph results and Master List for current Badge Level. Updates therapeutic progress with input from Clinical Staff
    • Modifies plans to reflect placement in IPMP, Suicide precautions or updated violence risks
    • Short Term Goals including timeframe dates that is estimated to be within resident's impending ability to meet and individualized to resident.
    • Updates Potential early warning signs of deterioration as instructed by Clinical staff
    • Patient and Family Involvement, documenting
    • Subjective content, including patient quotes
    • Objective content, including staff/patient discussion and goal achievement during last review period
    • Future goals identified during upcoming review period
    • Patient/family/guardian involvement in CITP
    • Assists the Treatment Team Facilitator with appropriate facilitation of the Team by keeping disciplines on task and focused and interruptions to minimum.
    • Promotes development of measureable short-term and accuracy of Progression towards long term goals.
    • Ensures CITP is written in language that the resident can understand and contains all required information
    • Notifies Supervisor when resident is dropped in Tier level
    • Completes Treatment Team Notes for Falls and Medical Emergencies
    Distribution of CITPS and Surveys


    • Sends email of CITP to those in attendance for signature, following up as needed to ensure signatures are secured.
    • Marks plan as complete
    • Prints final copy for resident
    • Distributes Patient/Resident surveys.
    Quality Assurance


    • Initiates and maintains approved flow sheet ensuring CITP due dates, assigned therapist, current unit are correct
    • Ensures CITPS are scheduled and updated according to policy CD-304
    • Maintains an EPN list for nurses noting 90-day time frames
    • Reviews policies as updated & training
    TPC's will fill in for Annual examinations via teams as needed on IR and Jung building.

    Qualifications


    • Minimum Qualifications: One year of experience in general office, clerical and administrative support work. Education may be substituted for experience as determined relevant by the agency.
    • Preferred Qualifications:
    • Post-Offer, Pre-employment Requirements: Ability to pass a KBI background and pre-employment drug test.
