Within our Community Connections Division, and in partnership with the Florida Department of Health, Human Services provides the Pinellas County Health Program, offering prevention focused health care, mental health and substance use services, specialty care referrals, and case management services to qualified low-income residents.

We are seeking a dynamic team player with a passion for helping others to fill our Case Manager 1 position.

This position will be located at 647 1st Ave N in St. Petersburg FL with a Monday - Friday, 8:00 a.m. - 5: 00 p.m. hybrid schedule including office, outposts and some remote work.


Essential Job Functions

  • Screens and interviews applicants in person and over the phone, and responds to questions regarding availability of, and eligibility for benefits.
  • Provides information and referral services to clients.
  • Works with community partners and other agencies to assist clients in accessing services and programs.
  • Answers phone calls from the general public to provide information and resources.
  • Maintains an active caseload for eligibility and shortterm case management.
  • Enters accurate and timely notes, and scans documentation and forms in appropriate databases.
  • Applies policies and procedures of the department's eligibility determination process.
  • Performs data entry and record keeping.
  • Prepares and prints medical ID cards and issues to clients.
  • Attends meetings and trainings.
  • Performs other related job duties as assigned.

Position Specific Requirements

  • Five (5) years of technical and professional experience in social/human services case management or related field; or
  • An Associate's degree in social work, psychology, sociology, or a related field and three (3) years of experience as described above; or
  • A Bachelor's degree in social work, psychology, sociology, or a related field and one (1) year of experience as described above; or
  • An equivalent combination of education, training, and/or experience.
  • A valid Florida Driver License, in good standing, will be required to travel to offsite locations and various trainings and meetings.
  • Assignment to work a variety of work schedules including compulsory work periods in special, emergency, and/or disaster situations.
  • Ability and willingness to travel throughout Pinellas County.
  • Participation in the Pinellas County Employee Disaster Preparedness & Response operations.

Highly Desirable

  • Knowledge of services offered by the County and other local community services agencies.
  • Fluent in any languages other than English, particularly Spanish.
  • Ability to work a flexible schedule when needed.
