This pivotal role requires close collaboration with all stakeholders, including individuals, families, and other agency team members, to create meaningful opportunities for people with disabilities to actively participate in their communities as valued neighbors, productive workers, and respected citizens.

You will complete and conduct assessments for new Participants to learn of skills, abilities, interests, and support needs for Community Employment.

Develops community business relationships for potential job development to secure employment. Provides individualized job support, negotiates work related needs, facilitates social integration and interdependence.

Be a liaison between client, counselor and community employer at time of hire, job orientation. Review potential job sites reviewing qualifications, work environment, managerial and staff attitudes. Fully documents visits with other involved staff. Identify barriers and finding solutions to work-related needs. Facilitate social integration and interdependence or natural support.

MUST be responsible, energetic, enthusiastic, compassionate & LOVE working with adults with I/DD

• Minimum of two years direct experience with individuals with I/DD
• Successful completion of required background checks
• Valid WI driver's license
• Excellent interpersonal and communication skills
• Independent problem solving skills

Staff and individuals served continue to adhere to recommended CDC COVID-19 precautions, ie focused on at-risk populations.


Expected hours: 35-40 per week

$ $18.50 per hour
