Responsibilities may include:
Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
Attend the 72 hour care plan meetingOrdering equipment for the community settingArranging home health nursing for after discharge, as neededCoordinating the medications for community useCoordinate the entire discharge, with IDT involvementAttending care conferences for sub-acute residents/guestsAttending Medicare MeetingsAssist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.
Initiate and coordinate completion of the Post Discharge Plan of Care Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
