    REMOTE POSITION HOWEVERMUST RESIDE IN VIRGINA TO BE CONSIDERED FOR THE POSITION.

    • Monday through Friday
    • 8:00am -5:00pm EST
    • Required Equipment (not provided)
    • Laptop/ Desktop // keyboard // mouse // additional monitor

    Summary:

    Case manager is responsible for health care management and coordination of healthcare members in order to achieve optimal clinical, financial and quality of life outcomes. Works with members to create and implement an integrated collaborative plan of care. Coordinates and monitors member's progress and services to ensure consistent cost-effective care that complies with Molina policy and all state and federal regulations and guidelines.

    Job Qualifications

    Must Have Skills:

    • Must have a valid driver's license and reliable vehicle for field visits
    • Excellent communication skills, with an ability to communicate effectively with members with chronic conditions, developmental disabilities, etc.
    • Strong technical skills (utilizing multiple programs at the same time) along with competency in Microsoft applications (Outlook, Teams, Excel, etc.)
    • Strong organizational and interpersonal skills
    • Bachelor's degree in Nursing or Masters degree in Social Work, or Health Education
    • Active, unrestricted State Registered Nursing license or Licensed Clinical Social Worker LCSW
    • 2 years of clinical experience with case management experience
    • Prior Authorization experience
    • Must have valid driver's license with good driving record and be able to drive locally.
