  • Provide assistance to client in the home and in the community
  • Delivering individualized care for residents in accordance with their Person Centered Plan
  • Advancing residents' decisionmaking, productivity and participation in a range of activities.
  • Promoting independence in daily activities, such as work, learning and leisure.
  • Facilitate client's inclusion in the community's experiences and activities.
  • Maintaining knowledge of each client's limitations and requirements.
  • Maintaining documentation in compliance with company procedures.
  • Transporting client's to and from appointments, volunteer opportunities and leisure activities
