- Conduct Supervisory Visits and Education in a Patient's Home
- Assess patients' skilled nursing/PSS needs
- Work independently with minimal supervision
- Maintain strict patient confidentiality
- Follow established policies and procedures for decision-making
- Prepare clinical progress notes
- Communicate changes in patients' condition to the family and Registered Nurse
- Administer medications and treatments as prescribed
- Provide total patient care
- Offer patient and family education
- Perform other services as outlined in the Georgia Licensed Professional Nurse Practice Act
- Report to the Registered Nurse and Clinical Director
- Must have a current unencumbered Nursing license as a Licensed Practical Nurse (LPN) in Georgia
- Hold current BLS Certification
- Possess a valid Driver's License and TB Screening
- Experience in caring for medically fragile individuals
- Preferred experience in Home Health and Pediatrics
- Familiarity with Georgia Pediatric Program, GAPP, policies and procedures (preferred)
