Job Description Summary


The Human Services Specialist II will function as a Peer Adherence Counselor, providing psychosocial support services to adult patients with HIV who are new to the Infectious Disease clinic and established patients struggling with medication adherence.

Work Shift

Job Duties:


  • 35% Work closely with patients who are new to the Infectious Disease Clinic and have a diagnosis of HIV. Reach out to new patients prior to their appointment. Ensure they know how to get to the clinic. Help patients navigate the new patient appointment, ensuring they see case management, social work, and the clinical pharmacist. Help the patient navigate to the lab and pharmacy before leaving after the appointment. Follow up with the patient in three to five days after the appointment to ensure the patient was able to get their medication and answer any questions they may have.
  • 35%—Work with patients who have problems with medication adherence. Monitor monthly missed refill reports from SC ADAP and Avita Pharmacy. Follow up with patients who did not refill medications for the month. Utilize Peer Adherence Intervention Tools that address 30day, 60day, and 90day missed refill events.
  • 15%—Work closely with medical case managers, specialized case managers, and outreach to monitor patients returning to care. Contact patients prior to the return appointment to answer any questions they may have. Help the patient navigate the returntocare appointment. Ensure followup appointments are made and the patient completes all tasks before leaving.
  • 15%—Maintain accurate documentation of services, contact information, and other patient demographics in Provide Enterprise database and Epic.

Additional Job Description

Minimum Requirements:
A high school diploma and two years relevant program experience. A bachelor's degree may be submitted for the required program experience.
