    Job Title: Physical Therapist

    Description

    The Physical Therapist provides prescribed treatments of physical therapy, designed to restore the patient to their maximum rehabilitative potential.

    This role involves identifying problems and performing physical therapeutic treatments as prescribed by the patient's physician.

    Responsibilities

    • Assesses, reports, and records patient status and progress on appropriate Agency laptop and to the health care team.
    • Ensures initial and ongoing assessments to determine level of functioning and evaluation of patient/family response to therapy, including OASIS assessments at appropriate time points.
    • Plans, evaluates, implements and teaches patients and family members to provide for a continuous physical therapy program during care and discharged.
    • Participates in clinical conferences and in-service programs to share information and stimulate the health care team in meeting rehabilitation goals.
    • Delivers quality treatment services to patients in accordance with accepted standards of practice and effective for the condition, which the patient suffers.

    Requirements

    • Graduate of an approved Physical Therapy curriculum.
    • One-year of experience as a Physical Therapist required.
    • Physical Therapist currently licensed to practice physical therapy in North Carolina.
    • Maintain current CPR certification (BCLS).

    Travel Required:

    Will be expected to travel to other offices to meet the demand of patient needs as instructed by Clinical Director.
