Founded in 1980, Neighborhood Self Help by Older Persons Project (SHOPP) is a non-profit social service agency located in the Bronx serving older adults and families caring for an ill, frail, or disabled loved one.

SHOPP's mission is based on the philosophy of self-help.

We believe that older adults are capable of helping themselves and helping others improve their capacity to deal with the physical, emotional, social, and financial challenges they face as they age.

SHOPP has several service programs that includes Community Care Network Program (CCNP); VIP Elder Justice; NY Connects of the Bronx; Case Management & Friendly Visiting Program; Caregivers Program, Lafayette Estates NORC Program;

Older Adult Centers:
Casa Boricua, Leon, Soundview, and Guess.

Program
(CSP) is a new initiative at SHOPP that supports formal and informal caregivers caring for an ill, frail, disabled, or cognitively impaired individual with Alzheimer's or other forms of Dementia or those 55 plus Kinship relation
- grandparent raising a grandchild(ren). The
CASE
MANAGER will provide client assessments, individual counseling, care coordination, educational training, respite services, supplemental assistance, information & referral, and outreach initiatives to existing and prospective caregivers. The Case Manager role is vital in ensuring that caregivers and their families receive the information, support, and resources they need to cope with the challenges of caregiving. This full-time position reports to the Director of SHOPP's Caregivers Support Program.

Skills


The Case Manager position requires a Baccalaureate Degree in Social Work or a related field from an accredited school with 1-2 years of work experience in a social service setting.

Knowledge and experience working with families are preferred.

Essential competencies, knowledge, and skills include advocacy, brokerage, care coordination, communication, problem-solving, collaborative teamwork, and managing a caseload under mínimal supervision.

Bilingual skills in Spanish are helpful. Computer literacy is mandatory, and knowledge of STARS is also helpful. The Caregiver Support Program follows a hybrid schedule with mandatory monthly home visits in the North Bronx. Conducting home visits may require walking up and down stairs and conducting fieldwork in different weather conditions.

All staff must be able to travel and report to the office at a minimum of 2-3x a week or otherwise requested, as well as attend in-person training, meetings, or events.

The
CASE MANAGER
is specifically responsible for the following:

  • Maintain client caseload of ongoing and short-term clients.
  • Perform intake assessments and create a plan of care for each client.
  • Conduct ongoing client assessments, determine service needs, and timely followup.
  • Perform home visits as needed.
  • Assist with program development and delivery.
  • Provide caregivers/care receivers with individual case coordination, counseling, and support services.
  • Assist, educate, and support caregivers.
  • Provide information, direction, and assistance with caregiving issues such as coordinating service needs, coping with caregiver burden, purchasing needed supplies, and managing the care receiver's health care.
  • Facilitate support groups and educational training.
  • Educate caregivers and/or care receivers in implementing advanced directives, such as guardianship, health care proxy, and power of attorney.
  • Assist and educate caregivers/care receivers on longterm care options such as Medicaid, home care, adult day, nursing home, etc.
  • Provide followup and advocacy assistance.
  • Maintain case records on a database, including documentation of all caregiver services provided as directed by NYC Aging.
  • Identify and maintain uptodate resources for client referrals.
  • Identify and collaborate with community partners to establish effective HUBs or stations in communities throughout the Bronx.
  • Collaborate and work with student interns and volunteers.
  • Help develop and maintain a resource file of caregiver support information.
  • Promote the agency and program by performing outreach, participating in community events, and representing the agency to the public. Develop and maintain liaisons with other organizations, as appropriate, to collaborate in outreach efforts or other partnership opportunities.
  • Help coordinate and host major events that appeal to the broader community and/or the professional network.
