    JOB TITLE: Case Manager

    Program: CHAP (Comprehensive Housing Assistance Program)

    Date Written/Revised: February 2022


    Basic Functions:

    To offer housing support, budget counseling, and case management to clients referred to our program, which is in partnership with Penn Medicine/Lancaster General Health.

    Essential Functions:

    • Mediate and advocate with landlords on behalf of clients to create housing solutions.
    • Assist clients in finding and using available housing aid programs.
    • Set up personalized goal plans with clients, based on thorough needs assessments and self-sufficiency objectives.
    • Facilitate adherence to budget and case management plans through housing and budget counseling sessions.
    • Establish and maintain communication among involved parties like tenants, landlords, and relevant social service agencies.
    • Provide information and referrals for support services.
    • Utilize knowledge of lease contracts to educate individuals about their rights and obligations.
    • Offer direct case management, home visits, and follow-up services to ensure housing stability and progress towards self-sufficiency.
    • Maintain accurate case notes and client files.
    • Ensure client confidentiality.
    • Transport clients as necessary within safety protocols.
    • Assist in preparing required reports as directed.

    Other Functions:

    1. Enter data into HMIS system.
    2. Keep comprehensive documentation of activities and paperwork.
    3. Take on additional assigned responsibilities.

    Supervision Received: Reports to Program Manager

    Minimum Knowledge, Skills and Abilities:

    • 2-year post-secondary education required, Bachelor's Degree in Social Work preferred.
    • 1 year of relevant experience necessary.
    • Bilingual in English and Spanish.
    • Strong communication, mediation, and writing skills.
    • Organizational and detail-oriented with ability to handle a demanding workload.
    • Commitment to Housing First Philosophy.
    • Knowledge of tenant's rights, case management best practices, and budgeting.
    • Familiarity with social service agencies and community programs.
    • Computer proficiency, especially in Microsoft Office suite.
    • Belief in empowering others, fostering growth, and building trust.
    • Collaborative work ethic with service providers.
    • Valid driver's license and reliable transportation.
    • Willingness to travel throughout Lancaster city and county.

    Classification: Regular Full-Time Employment - full-time staff eligible for benefits after meeting requirements per Personnel Policies Handbook.
