AFA History:

AFA was founded in 2002 by a family caregiver to ensure families affected by Alzheimer's disease always had a place to turn for guidance, support, and services.


AFA Mission Statement:

The mission of the Alzheimer's Foundation of America (AFA) is to provide support, services and education to individuals, families and caregivers affected by Alzheimer's disease and related dementias nationwide, and fund research for better treatment and a cure.

Responsibilities:
_**- Link callers and their families to appropriate services and programs.

  • Assess and address safety and/or risk and develop safety and intervention plans accordingly.
  • Facilitate support groups for those affected by dementia, either via phone or zoom.
  • Participate in educational offerings, such as providing webinars and facilitating group forums on topics related to dementia.
  • Assist with special projects and other duties as assigned by the Director of Educational and Social Services.

Qualifications:


  • Master's Degree in Social Work or related field (Psychology, Counseling, Mental Health or Gerontology) and NY State Licensed.
  • A minimum of 1year relevant work experience with the elderly population, caregivers and health professionals in the field of dementia and Alzheimer's care.
  • Excellent interpersonal, listening, verbal and written communication skills.
  • Excellent and continuous follow up skills a must.
  • Must be available on evenings and weekends per a flexible work schedule.

Preferred:


Proficient language skills in Spanish, Cantonese, Mandarin or another language
