- Teach and demonstrate to patients and/or families nursing procedures in care and rehabilitative measures
- Provide continuity of care by referral to appropriate resources for health needs and follow-up on referrals
- Participate in the development of a comprehensive community health nursing program which is based on assessed community needs and objectives related to maternal child health, chronic disease, communicable disease control, health promotion and disease primary prevention, health education, case finding and case management.
- Conduct visits to the home, community, school and clinic to provide community health nursing services, including health teaching.
- Provide in-service education and other training to staff relative to public health issues and serves as a resource person by identifying and collecting health care and health status data.
