    The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers.

    This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest.

    As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.

    This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community.

    This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.


    The Advanced Practice Nurse (APN) is a Registered Nurse who has a master's, post-master's certificate or practice focus Doctor of Nursing Practice degree in one of four specific roles and provides patient care within their role and patient population focus.

    The Advanced Practice Nurse (APN) functions directly to support the activities of Primary and Specialty Medicine (PSM). The APN is responsible for the planning, integrating, implementing, modifying, and evaluating the effectiveness of the Veteran's care. The PSM foundational elements will serve to support delivery of high quality, timely care to the Veteran receiving services. The APN is the lead of the integrated care team established by the PSM. He/she is responsible for providing clinical leadership and expertise on PSM processes and regulations.

    The APN will serve as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community.

    He/she possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy related to Delegation of Authority as established by the facility Chief of Staff.

    Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.

    The nurse's primary commitment is to the patient, whether an individual, family, group, or community.

    Work Schedule: 8:00 AM to 4:30 PM, Monday through Friday

    Pay:
    Competitive salary, annual performance bonus, regular salary increases

    Paid Time Off: 50-55 days of annual paid time offer per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year and possible 5 day paid absence for CME)

    Qualifications


    Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.


    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Certification: Family Nurse Practitioner

    VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

    Physical Requirements:

    Heavy lifting (45# or more), moderate carrying (up to 45#), reaching above shoulders, use of both hands and legs, use of fingers, prolonged walking and standing, intermittent crawling, kneeling, and bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16 to Jaeger 1 to 4, far binocular correctable to 20/40, vision, hearing (aide permitted), mental and emotional stability.

    Additional information


    Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
