Description
Provides clinical assessment, service coordination, therapeutic interventions, and discharge planning for the residential program.

  • Assessment of individuals with substance use disorders with treatment recommendations provided to treatment team and other service providers.
  • Collaborates with internal and external treatment providers to ensure appropriate treatment recommendations and discharges plans are in place.
  • Conducting individual and family sessions, group therapeutic activities, and discharge planning, as needed, for adults who present for service.
  • Responsible for accurate and timely documentation of all information in the clinical record.
  • Actively respond to individuals who show signs of crisis, which may include mental health symptomology or unsafe behaviors. Work as a team with other staff to deescalate.
  • Assists with screening referrals for appropriateness and coordinating admissions
  • Communicate admission to agency staff, programs and / or community partners who are currently involved with patient.
  • Develop a behavioral treatment plan with measurable objectives that the individual can work toward while completing residential treatment.
  • Complete chart audits while updating and monitoring accuracy of clinician tracker.
  • Bachelor's degree or higher in human services field.
  • Minimum one (1) year experience working with individuals in behavioral health.
  • Ability to pass a level II Background clearance and drug test.
  • Valid FL drivers' license, insurance, and safe driving record.
  • Ability to lift and move objects weighing up to 25 pounds
  • Stamina to stand, walk, sit, and perform tasks for extended periods.
  • Manual dexterity and visual acuity for documentation.
  • Auditory acuity for verbal interactions whether facetoface or over the phone.
  • Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
  • Ability to use a computer

  • Ability to work independently and as part of a team, in collaboration with other community partners.
  • Ability to manage stressful situations and display appropriate work demeanor and boundaries.
  • Knowledge of community resources and ability to effectively communicate with community organizations.
