Description
The Care Coordinator functions as a vital role in facilitating provision of quality care coordination of all patient services.

The following serves as a representative summary of the major duties and responsibilities of the Care Coordinator position, however duties are not limited to what is stated in this general description.

This position is Monday-Friday from 8am-5pm within the Office.


ESSENTIAL JOB FUNCTIONS:


  • Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
  • Data entry of referral information into electronic medical record system, including patient demographics, diagnosis, medications, and specific orders related to patient care.
  • Coordinates to schedule Initial consults and routine visits.
  • Coordination of daily schedules by working closely with team of nurse practitioners.
  • Manages on-call schedule for field staff and ensures adequate coverage of on-call responsibilities
  • Manages tracking of lab results and reports the required results to nurse practitioners.
  • Ensures provision of quality care coordination through scheduling tasks and sending wound care updates to collaborating disciplines.
  • Obtains and writes physicians orders, including appropriate scheduling of the requested task.
  • Establishes and maintains positive working relationships with current and potential referral sources
  • Acts as a back-up to Office Manager in their absence

Position Qualifications:


  • At least one year experience in health care office setting, preferably in home
care/hospice operations


  • Knowledge of use of Microsoft Office, Zoom, EMR systems, E-fax, Copy, etc
3.. Knowledge of governmental regulations, CMS, CHAP standards, and private payer practices


  • Demonstrates good communication, negotiation, and public relations skills
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
  • Organized and thorough communicator.

Pay:
Up to $50,000.00 per year
