    Join our team at UPMC Presbyterian Shadyside as a Discharge Plan Associate!

    If you are an RN or social worker with a passion for care management, case management, or care coordination, we want you to be a vital part of our Clinical Care Coordination and Discharge Planning department.

    Your role as a Discharge Plan Manager

    • Assist in the safe transition of patients to their homes or other care settings.
    • Work closely with healthcare providers, patients, and families to create personalized discharge plans.
    • Advocate for patients' needs and preferences throughout the discharge planning process.

    Responsibilities:

    • Work in accordance with system-wide competencies/behaviors.
    • Advocate for patient's services access and health protection.
    • Maintain clinical knowledge and ensure compliance with regulations.
    • Complete detailed patient assessments and develop discharge plans.
    • Communicate effectively with patients and the care team.
    • Facilitate safe and efficient discharges.
    • Ensure appropriate post-hospital care arrangements.
    • Coordinate individualized plans of care.
    • Integrate patient goals and healthcare team assessment.
    • Evaluate patient and family understanding and engagement.
    • Recognize social determinants of health impacting transitions.
    • Demonstrate shared accountability in plan development.

    Experience in Discharge Planning:

    • Coordinate clinical care needs for various transitions.
    • Organize resources and support for patients' transition.

    Nurse Track Requirements:

    • Diploma or Associate's Degree.

    Social Worker Track Requirements:

    • Bachelor's degree in social work or a related field.
    • No license required.

    Licensure, Certifications, and Clearances:

    Nurse track:

    • RN License required.

    Social Worker Track:

    • No license is required.
