Department Description
The role of the Home Infusion Nurse Liaison is to provide exemplary infusion care to patients.

Home Infusion RNs are experts in vascular access care and employ a wide range of techniques to ensure patients receive an exceptional care experience.

Home Infusion RNs provide standard and progressive infusion therapies such as antibiotics, total parenteral nutrition, immunotherapies, enzymes, and other specialty medications that require the knowledge and skills necessary to respond to infusion reactions autonomously in the home.


With a focus on patient/caregiver teaching and care coordination, the clinical liaison ensures a safe transition for patients from hospital to home.

The clinical liaison works primarily in the hospital and may perform infusions at the Health at Home infusion suites and also in the home setting.


The position requires some travel to patient's homes and administration of intravenous and subcutaneous infusions to support patients' health goals.

The HH RN 2 provides direct patient care to a diversified patient population.

Applies knowledge, experience and judgment to determine the importance of a situation, sets priorities, and uses abstract thought to evaluate clinical situations.

Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Minimum Qualifications

  • Graduation from an accredited school of nursing.
  • 1 year of Home Infusion Pharmacy experience.
  • 1 year of experience administering specialty IV medications including biologics.
  • 2 years of Vascular Access Device experience including accessing, maintaining, and blood sampling.
  • 2 years of experience placing peripheral IVs and performing peripheral blood draws
  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) BLS CPR Certification
  • Advanced assessment skills required to recognize early signs and symptoms of an infusion reaction and take necessary steps to prevent patient harm.
  • Ability to administer firstdose medications in the homesetting.
  • Knowledge and ability to administer anaphylactic medications in an emergency.
  • Ability to start peripheral IVs in patients with varying vasculature.
  • Ability to admix medications in the homesetting.
  • Demonstrated ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients.
  • Possess exceptional verbal and nonverbal communication skills, and present with an open and adaptable demeanor when faced with challenges.
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.

Preferred Qualifications

  • Bachelor of Science in Nursing
  • Inpatient bedside nursing experience
  • Infusion clinic experience
  • Oncology experience
  • Certified Registered Nurse Infusion (CRNI)
  • Vascular Access-Board Certified (VA-BC)
  • Oncology Certified Nurse (OCN)
  • Pediatric Nursing Board Certification (PED-BC).
