    Job Description

    The Behavioral Health Specialist (LICSW) is a valuable member of the primary health care team, focusing on addressing the psychosocial and substance abuse needs of patients at corporate locations. Actively seeking out individuals, especially those in special population groups, who require behavioral health services.

    Principal Duties and Responsibilities:

    • Establishes and maintains relationships with external organizations to build effective mental health networks for special population groups.
    • Conducts timely assessments on referred patients.
    • Creates personalized mental health, substance abuse, and/or social service plans based on assessments.
    • Collaborates with provider staff on referral policies and patient care management.
    • Assists in developing case management guidelines and referral methods for seamless patient care.
    • Engages in mental health/substance abuse prevention activities in the community.
    • Coordinates care with other primary care team members.
    • Conducts individual, family, and group counseling sessions as necessary.
    • Identifies and manages mental health and substance abuse resources to meet patient needs.
    • Maintains up-to-date and confidential case records for each patient.
    • Ensures patient and corporate information confidentiality.

    Qualifications:

    • Master's degree in Social Work, Counseling, or related human science field required.
    • Preferably holds a State of Alabama license in the relevant discipline.
    • Minimum of two years of relevant experience, ideally in a medical environment.
    • Proficient in basic computer skills.
