Overview:
We are seeking a dedicated and compassionate Memory Care Coordinator to join our team. As a Memory Care Coordinator, you will play a crucial role in managing and coordinating care for our residents. This position requires experience in senior care and memory care.

Responsibilities:

• Coordinate and supervise the delivery of care services to residents
• Develop and implement care plans based on individual needs
• Monitor resident progress and make adjustments to care plans as necessary
• Provide guidance and support to caregivers and nursing staff
• Ensure compliance with resident rights and maintain confidentiality of resident information
• Collaborate with healthcare professionals to ensure optimal resident care
• Assist with medication administration and ensure proper documentation
• Conduct assessments and reassessments of residents' physical, emotional, and cognitive status
• Provide education and support to residents and their families regarding care options

Experience:

• Previous experience in nursing or senior care is required
• Knowledge of dementia care principles and practices is preferred
• Strong organizational skills with the ability to manage multiple tasks simultaneously
• Excellent communication skills, both written and verbal
• Ability to work effectively as part of a team

Pay:
From $25.00 per hour
