Description

StarCare Specialty Health System (StarCare) seeks to hire a Rural Diversion Aftercare Specialist in Hockley County (Levelland).

Come join an elite team of mental health professionals providing services and supports to those living in rural Texas. Work is performed under the general supervision of the Rural Diversion Team Lead and Crisis Services Director, with moderate latitude for the use of initiative and independent judgment. This is a field-based position in Levelland/Hockley County.

Aftercare Specialist Responsibilities:

• Provide follow-up services to individuals assigned to Level of Care 1 and Level of Care 5.
• Provide 7-day follow-up for individuals living within Hockley County discharged from Sunrise Canyon Hospital, all contracted hospitals, as well as discharges from state hospital facilities to ensure proper continuity of care.
• Provide 24-hour follow up for all individuals with an initial crisis contact living within Hockley County as well as providing appropriate 7-day follow up services to individuals opened to Level of Care 0 to stabilize crisis situations in the least restrictive environment possible.
• Assist Mobile Crisis Outreach Transition Specialist as needed to ensure appropriate follow up for patients assessed by the Mobile Crisis Outreach Team (MCOT) and discharged of psychiatric inpatient units.
• Primary focus will be the City of Levelland and Hockley County.

Schedule: This is a full time / non-exempt position working Tuesday - Friday, 8:30am to 6:30pm with flexibility to meet the needs of the program.

Qualifications

Qualifications:

• Graduation from an accredited four (4) year college or university with major course work in a Human Services field (Human Development/Family Studies, Psychology, Sociology, etc.) with two (2) years full-time experience in a related field. preferred.
• Crisis Experience, preferred.
• Bilingual (English/Spanish), preferred

Knowledge, Skills & Abilities:

• Knowledge of human service delivery systems.
• Ability to communicate effectively, both orally and in writing.
• Ability to organize, prioritize a variety of assignments and manage time effectively.
• Ability to maintain an effective working relationship with other employees and the public.
• Knowledge of appropriate rules, regulations, and policies and procedures

Additional Requirements:

• Must pass a pre-hire drug screen and criminal background check.
• Must have a Texas driver's license and be insurable under the Agency's insurance.
• Local travel is required. Must have reliable transportation. Mileage will be reimbursed at state approved rate.
