- Complete aftercare follow ups with consumers discharging to our catchment area from all hospitals, including Sunrise Canyon.
- Provide follow up services to those placed in 7 day crisis services along with providing skills training and routine case management such as completing treatment plans to those authorized for SP5 transitional services.
- Assist in scheduling appointments for those discharging from the Extended Observation Unit.
- Complete crisis assessments and act as an additional responder for MCOT during peak times which includes answering on call phone when needed, assisting with hospital transfers when needed and responding to all crisis situations in an appropriate manner.
- Assist MCOT supervisor with additional task deemed appropriate in order to meet program needs.
- This is a Field Based position.
- Graduation from an accredited four (4) year college or university with major course work in a Human Services field (Human Development/Family Studies, Psychology, Sociology, etc) with two (2) years full-time experience in a related field
- Master's Degree in a Human Service related field with applicable licensure to include: Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Master of Social Worker-Advanced Clinical Practitioner (LMSW-ACP), Licensed Clinical Social Worker (LCSW) preferred.
- Crisis Experience preferred.
- Bilingual (English/Spanish), preferred.
- Must pass a pre-hire drug screen and criminal background check.
- Must have a Texas driver's license and be insurable under the Agency's insurance.
- Master Level Restricted LPHA (Licensed Practitioner of Healing Arts): $1 per hour
• - Master Level Unrestricted LPHA: $6 per hour
• - Night shift: $1 per hour (Night differential is applied to hours worked between 8pm and 8am)

StarCare Specialty Health System (StarCare) is seeking a full time Transitional Specialist to join its outstanding Mobile Crisis Outreach Team (MCOT) supporting people with mental illness at the Lubbock County Detention Center.

StarCare's Mobile Crisis Outreach team is a highly trained team dedicated to serving those in mental health crisis within the 5 county catchment area served by StarCare. StarCare is the local mental health authority that services Lubbock, Hockley, Lynn, Cochran, and Crosby counties.

Responsibilities:
