The Patient Care Manager (PCM) is responsible for overseeing the care provided to assigned team patients, focusing on leadership and management of the associated team consisting of Clinical Support Specialists and Field Clinicians.
Ensure updated schedules, continuity of care, and delivery of quality care by the clinical care team.
Facilitate case conferences and follow-up actions related to meetings.
Mentor clinicians and CSS role, addressing development needs and holding them accountable to role-specific KPIs.
Support and find solutions to potential personnel concerns.
Supervise and direct team members, assign visits, and ensure timely completion of visits.
Manage the site in the absence of the Clinical Manager, Executive Director, or Administrator.
Oversee clinical, administrative, and recordkeeping functions.
Respond to phone referrals and inquiries professionally and compassionately.
Participate in on-call nursing schedule and provide skilled bedside nursing care in patients' homes.
Requirements of Qualified Candidates:
Registered nurse with current licensure in the practicing state.
CPR certification.
Previous experience in a home care setting with two years of management or supervisory experience preferred.
Ability to work within an interdisciplinary setting.
Licensed driver with insured and in good working order automobile.
Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements.
