Home Care RN Case Manager


A home care nurse has a strong sense of compassion and the ability to assess and provide skilled care in where the patient feels most secure, their home.

A home care RN case manager plans and coordinates home health services to meet the needs of individuals and families within their homes and communities, and is experienced in all aspects of skilled nursing.


Duties include but are not limited to:

  • Completes an initial assessment of patient and family to determine home care needs.
  • Follows up with a complete assessment, development of an individualized written plan of care for each assigned patient.
  • Initiates the plan of care and assumes responsibility for the
    ongoing interdisciplinary assessment and expansion of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Assesses patient's condition each visit, re-evaluates and updates plan of care as necessary.
  • Initiates proper skilled preventive and rehabilitative nursing procedures and provides services as ordered by the physician and indicated in the plan of care.
  • Counsels and provides education to the patient and family in meeting nursing and related needs.
  • Prepares thorough clinical notes.
  • Communicates with the physician who is responsible for the home health plan of care and other health care practitioners (as appropriate) related to the current home health plan of care.
  • Obtains all necessary physician orders and collaborates with physician as patient's needs dictate.
  • Completes at least 75% of all charting while in the patient's home.
  • Attends case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Re-evaluates the need for continued care on an ongoing basis, initiates recertification as necessary.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

Additional Duties

  • Participates in the home health agency's quality assessment and performance improvement program and home health agency-sponsored in-service training.
  • Participates in holiday rotation and on-call duties as defined by the on-call policy and agency requirements.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises, and evaluates home health aide care and LPN care provided every two (2) weeks per state and agency regulations.
  • Participates in all mandatory meetings, training, and in-service education.
  • Completes all weekend/holiday visit documentation by the following business day by 8:00 AM.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)


  • Graduate of an approved school of nursing and licensed in the State of Wisconsin.
  • Minimum of two (2) years of experience; one (1) year of public health or home care nursing preferred.
  • Must be a licensed driver with an automobile in good working order and insured in accordance with state or organization requirements.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills and nursing skills; organized and self-directed.

Pay:
From $38.00 per hour
