Description
Responsibilities include but are not limited to:
- Escort residents safely to and from appointments
- Observe and monitor residents
- Reporting any health issues to the supervising nurse
- Maintain confidentiality of all Resident & Facility data
- Assist with collection and distribution of resident laundry
- Assist family members in completing Patient Personal Property List Forms; delivering packages, such as gifts, flowers, clothing, to resident room or notifying housekeeping
- Assist residents with daily feeding/meals
- Coordinate and assist with resident room changes including logging and packing of personal belongings
- Assist with additional non clinical resident care duties as needed.
REQUIREMENTS:

Valid New York State HHA Certification (available upon request)
- Periods of standing for prolonged periods of time
- Ability to stoop or kneel
- Occasional heavy lifting may be required
- Ability to follow the Plan of Care precisely
- Must be able to work as a team member
- Flexible availability is required, including working weekends and holidays
- Valid photo ID required
- Must have had a physical within the last 12 months
- Proof of PPD and immunizations with the last 12 months
- Pass background check
