The Behavioral Health Navigator [BHN] is a healthcare extender that guides patients through and around barriers in the complex healthcare system.

As a health professional, the BHN will provide support to overcome obstacles faced in accessing or receiving services needed to maintain well-being & independence.


The BHN will provide practical support services to older adults with complex medical and mental health conditions, frail seniors, people with disabilities, and/or seniors facing homelessness.

The main function of this position is to facilitate patients' access to services that will increase health outcomes.


Essential Duties and Responsibilities:


  • Provide supportive services in patients' place of residence.
Advocate for patients by accompanying patients in various settings: medical offices, specialists, hospitals, and patients' homes.

Assist with developing patient care plans that reflect individual patients' needs and connect patients to appropriate resources as developed by Case Manager's treatment plan.

Coordinate patient care, e.g. transportation arrangements, medical appointment reminders, referrals, medication management, preventive care education, and/or follow-up care adherence.
Work with patients facing homelessness, substance abuse, mental health issues, and/or complex chronic medical conditions.
Maintains spreadsheet of all related data tracking for BHN position
Performs other duties as assigned.


Job Requirements:


  • BSW preferred.
Associated degree in Social Services or 3-5 years of related experience
Work experience in community-based health care service capacity
Excellent Communication Skills
Ability to manage multiple tasks.
Willingness and interest to work in the Tenderloin and other neighborhoods on behalf of low-income older adults.
Proficient in Microsoft Office Suite
Bilingual preferred Comfortable working independently on-site in clients' homes.
Excellent planning, organizational and facilitation skills.

Proof of Covid Vaccines required:
completed original series and first booster.
Proof of negative or nonactive TB required.

Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties.

Proof of a negative TB test within 90 days will also be accepted.

Strong computer skills such as Zoom, Chrome, Google Suite, Apple iOS, Office suite.


Physical Demands
Work will be performed sitting at a desk/chair or standing while speaking and interacting with clients. The position will require average finger dexterity to operate normal office equipment and systems.
