SUMMARY:


This position provides case management and service facilitation to children who have significant disabilities and their families, who are involved in Children's Long Term Support waivers (CLTS), Children's Community Options Program (CCOP), and Katie Beckett Program.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


The following duties are illustrative and the person holding this position may be required to perform other duties as assigned.


  • Perform all duties and tasks essential to the function of the Children's Long Term Support waiver, Children's Community Options Program and Katie Beckett Program.
  • Assess eligibility and need.
  • Coordinate evaluations and assessments. Facilitate and participate in development, review, and evaluation of the ISP and plans of care.
  • Facilitate development of individualized plans and transition plans; monitor plan implementation.
  • Provide case management for assigned cases. Prepare, execute, monitor and review case plans. Complete necessary requirements for recertification and eligibility in the various programs. Coordinate care with other service providers.
  • Provide information to families regarding available resources for children with severe disabilities and assist families in maximizing use of appropriate resources.
  • Make referrals to community or other resources when appropriate.
  • Responsible for making suggestions to improve program operations, existing service delivery systems, service selection and coordination. Responsible for identifying problems and barriers and recommending appropriate solutions within the context of the goals, objectives and constraints of the Department.
  • Maintain agency records and record case data on computer system. Submit reports as required within established time frames.
  • Provide community education and outreach regarding children and families as well as preventionoriented services.
  • Participate in unit meetings, staff meetings, supervisory conferences, and case conferences concerning case planning and provision of services within as well as outside the department.
  • Work using a team approach, with informal or formal teams, with other members of the department and representatives of other agencies, schools, and institutions to provide integrated services.
  • Participate in staff development, training programs, or other inservice programs as directed.
  • Adhere to confidentiality standards
  • Perform other duties and responsibilities as assigned.

REQUIRED QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Bachelor's degree in social work, sociology, psychology or another human services related field and some experience demonstrating aptitude for work with children with disabilities and their families.
  • Requires successful completion of the Wisconsin Functional Screen certifications.
  • Requires a valid Wisconsin driver's license and access to insured transportation.
  • Applicants must pass background checks.

  • Knowledge of available local, state and federal resources that will enhance the ability to provide case management.
  • Understanding of the special problems inherent to various disabilities.
  • The ability to communicate with, and recognize the needs of, persons with disabilities. Awareness of the responsibilities and the rights of those who have a disability.
  • Ability to work effectively with individuals and with teams to accomplish goals.
  • Ability to organize and prioritize time and activities to meet the needs of the workload.
  • Maintains regular and predictable attendance.
  • Promotes, gets along, and works in a harmonious relationship with others including but not limited to outside agencies and county departments.
  • Ability to read, analyze, and interpret common technical journals, government regulations, and legal documents.
  • Ability to effectively, concisely, and accurately convey information by written report or correspondence or orally in informal or formal settings, including court testimony.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.
  • Basic computer skills, including the ability to use a personal or laptop computer for preparation of written reports and correspondence, as well as data entry and use of specialized software.
  • Ability to work with mínimal supervision in a fastpaced and stressful environment.
  • Ability to work a flexible schedule based on the needs of clients
