- Bachelor's degree from an accredited college/university in an applicable human services field. Or relevant education, which indicates that the individual possesses the required knowledge, skills, and abilities.
- A minimum of one year of relevant mental health, developmental disability or substance use work experience or a combination thereof that demonstrates the candidate possesses the required knowledge, skills and abilities, and/or an equivalent combination of education, training and experience that provides the individual the required knowledge, skills and abilities.
- Services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disability, and substance use treatment programs;
- The nature of serious mental illness, developmental disabilities, and substance use depending on the population service, including clinical and developmental issues;
- Different types of assessments, including functional assessments, and their uses in service planning;
- Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination;
- The service planning process and major components of a service plan;
- The use of medications in the care or treatment of the population served;
- All applicable federal and state laws, state regulations, and local ordinances;
- Identifying and documenting an individual's needs for resources, services, and other supports;
- Using information from assessments, evaluations, observation and interviews to develop individual service plans;
- Identifying services and resources within the community and established service system to meet the individual's needs; and documenting how resources, services, and natural supports, such as family, can be utilized to achieve an individual's personal habilitative/rehabilitative and life goals;
- Coordinating the provision of services by public and private providers;
- Work as team members, maintaining effective inter-and intra-agency working relationships;
- Work independently, performing position duties under general supervision;
- Engage and sustain ongoing relationships with individuals receiving services;
- To drive and must possess a valid driver's license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client at other locations. Regular attendance at trainings and Horizon meeting may require travel to a variety of Horizon sites.
- To participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized as needed;
- Willing and able to work modified schedules (weekends, holidays, etc.) as required.
- Demonstrate a positive regard for individuals and their families (allowing risk taking, avoiding stereotypes of developmentally disabled people, respecting individuals' and families' privacy, believing individuals can grow).

Position Summary
The TREE Grant Case manager promotes and supports individuals who are at risk of developing or have mental health and/or substance use issues and works with individuals and families with at least one member with a substance use and/or mental health disorder to establish appropriate linkages; referrals; coordination of care; and provides regular follow-up on needed services and supports. Overall services include assessing, monitoring, linking to services, supportive level counseling, education, and advocacy. This is a grant-based position that will end with the end of the grant.
Required Education and Experience for Clinician – LMHP-E; Resident in Counseling
Related Knowledge, Skills and Abilities
Experience providing services (e.g., assessment, supportive level counseling, linkage and referral to sources) to adolescents and transitional-age youth. Familiarity with the culture and language of adolescents and transitional-age youth as evidenced by annual cultural competency training and 8 hours of annual trauma-informed training as documented in Horizon's learning management system. Access to language services for clients that communicate in a different language.
