    Position: Community Health Worker - Bridges to Moms Programs

    We are currently looking for a passionate full-time Community Health Worker (CHW) to join the Bridges to Moms Program at BWH. In this role, you will play a crucial part in supporting Obstetrics patients who are experiencing homelessness or are at risk of becoming homeless. Your responsibilities will include providing comprehensive care and assistance throughout the baby's first year.

    Duties and Responsibilities:

    • Offer community health services to pregnant and post-partum patients in the program
    • Close gaps in community resources related to Social Determinants of Health (SDOH) needs
    • Collaborate with patients and providers to set and achieve health goals
    • Provide culturally sensitive services to patients from diverse backgrounds
    • Help patients manage their care effectively by developing support systems
    • Assist with logistical barriers to care such as scheduling and transportation issues
    • Accompany patients to appointments and advocate on their behalf
    • Communicate regularly with providers and care teams
    • Refer patients to additional services when needed
    • Assist patients in filling out applications for public services
    • Contribute to program reports and attend training programs

    Qualifications:

    • Minimum two years of relevant work experience or Associates Degree
    • High school degree required

    • Knowledge of community-based services and resources
    • Excellent communication skills
    • Ability to connect authentically with individuals from diverse backgrounds
    • Strong attention to detail and organization
    • Willingness to learn service approach models
    • Strong time management and planning skills
    • Ability to collaborate with multidisciplinary teams
    • Proficiency in computer skills
    • Fluency in English and Spanish or Haitian Creole
