Description

• Assist with planning, developing, and directing social service programs in accordance with current federal, state and local standards, guidelines and regulations
• Ensure that medical, emotional, and social needs of each resident is met/maintained
• Greet and orient new residents
• Provide information regarding community resources to residents' families
• Coordinate discharge planning
• Facilitate regular care plan meetings with residents and their families
• Complete the MDS (Minimum Data Set) within the Assessment Reference date
• Address residents' concerns and assist nursing or other departments in resolving issues
• Attend/facilitate intervention, rehabilitation, and Medicare meetings as appropriate
• Other duties as assigned

• Bachelors Degree in Social Work, Psychology, Sociology, or a related health services field
• Must be able to work with residents and their families as needed
• Knowledge of all Social Service paperwork including assessments, documentation requirements, the MDS, etc.
• Familiar with discharge planning
• Must be able to work in collaboration with the interdisciplinary team
• Excellent communication and written skills
• Computer literate
