*Full Time Physical Therapist Case Manager**Anticipated Territory: Multiple Opportunities, Small Coverage areas in SF\**In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environmentYour goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Salary Description :
Wage range Physical Therapist, Level 1: 120, ,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. Generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match). As a practice, the majority of PT Case Managers are hired as PTIs.
Once it is determined that an PTI can supervise 2 PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to an PTII with a minimum salary of 135,000 plus 20K bonus (Paid Quarterly).
\* _You will be assigned to a County_ _as a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area._Job Type:
