Nurse Practitioner

QUALIFICATIONS:


The Nurse Practitioner shall be a Registered Nurse duly licensed to practice as an Advanced Practice Nurse in the state of licensure under the provisions of the State Board of Nurse Examiners and accreditation from an approved certifying body for Advanced Practice Nursing (American Association of Nurse Practitioners or American Nurse Credentialing Center).

It is preferred that the Nurse Practitioner has had professional home care and hospice experience.


RESPONSIBILITIES:

The primary objective of the Nurse Practitioner is to assist in oversight of the clinical care and treatment of patients served by Scenic Healthcare Hospice, LLC dba:

Texas Life Hospice - Fort Worth, and to consult and cooperate with the patients' attending physician, if any, and in collaboration with the Hospice Medical Director.

The Nurse Practitioner works in conjunction with the Hospice Medical Director and any other members of the delivery team to:


  • Provide clinical care and management to the patient to meet their medical needs within a reasonable timeframe in accordance with patient's plan of care.
  • Serves as an extension to the patient's attending physician ensuring that the plan of care is implemented as successfully as possible to promote the patient's comfort and safety in their environment.
  • Provide direct medical care as needed to patients, ensuring collaboration and coordination with the attending physician, and other providers, in the care of the patient seeking opportunities for improvement both in terms of outcomes and efficiencies.
  • Assists in developing the plan of care for each patient/caregiver and assures the quality of comprehensive plans of care with other members of the care delivery.
  • Reviews the quality of symptom management and modifies plan as needed in collaboration with the attending physician, patient, family and members of the care delivery team.
  • Communicates effectively, timely, and appropriately at all times.
  • Adheres to company policy regarding protected health information.
  • Assures the accuracy and timeliness of documentation in compliance with CPT codes, face to face documentation and clinical notes.
  • Ensure patient care is delivered at the appropriate level of care with coordination of PCP and medical director, provide management of medication profile to prevent polypharmacy and ensure optimal symptom management
  • Utilize the medication formulary whenever possible for the management of patient symptoms
  • Practice a high level of ethics and professional standards and be responsible for knowing and adhering to Company policies and procedures.
  • Follows and adheres to all federal, state and accrediting nursing standards and practices
  • Be capable of using Electronic Medical Records.
  • Ensure cost effective practices

Texas Life Hospice - Fort Worth within the professional and lay community.

  • Other duties as assigned by Scenic Healthcare Hospice, LLC dba:
Texas Life Hospice - Fort Worth administrative staff and Medical Director.

  • Participate in organizational committees and their functions if requested to.
  • Participate in QAPI activities as requested.
  • Participates in Community Education
  • Implements practices that ensures Family Satisfaction measures exceed national benchmarks

ORIENTATION AND TRAINING:


Scenic Healthcare Hospice, LLC dba:

Texas Life Hospice - Fort Worth will provide orientation and training, as necessary, to assist the Nurse Practitioner in performing the responsibilities listed above.

The Governing Body, Administrator, and the Management Team will act as a resource for integrated services available within the parent organization.

Trainings may be in-house or outside seminars or approved self-study and research. Documentation of orientation and training will become part of the personnel file.
