- Evaluate the patient's emotional and social well-being in relation to their medical condition, care needs, and response to treatment.
- Assess the patient's support system at home, financial resources, and available community services.
- Take necessary steps to access community resources that can aid in overcoming barriers to the patient's recovery.
- Educate patients and families on managing and coping with the emotional aspects of their medical condition. Provide ongoing assessment of their needs and response to education.
- Collaborate with the healthcare team to understand the social and emotional factors impacting the patient's health. Contribute to the development and periodic review of the patient's care plan.
- Monitor and report changes in patient status and treatment response. Participate in discharge planning.
- Participate in care coordination activities as part of the interdisciplinary team. Serve as a resource to address patient needs.
- Supervise, guide, and assess the performance of the Social Work Assistant to ensure the delivery of medical social services in compliance with standards.
- Maintain accurate documentation as per company and facility requirements, including case conferences, patient interactions, and collaboration with other healthcare providers.
- Master's or doctoral degree in social work from a Council on Social Work Education-accredited institution.
- Licensure as a Social Worker in the practicing state, if mandated by regulations.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Minimum of one year of social work experience in healthcare, preferably in home health or hospice settings.
- Knowledge of discharge planning and ability to access community resources.
- Strong communication and interpersonal skills.
- Proficiency in English (reading, writing, speaking).
- Familiarity with medications and proper administration.
- Ability to organize tasks, prioritize, and work effectively under pressure.
- Flexible availability for work hours and local travel.
- Effective communication with patients, families, and colleagues.
- Maintain licensure and meet continuing education requirements.
- Punctuality and regular attendance.
- Perform additional duties as assigned.
