North Westchester Restorative Therapy and Nursing has an opportunity for a Neurological Rehabilitation Coordinator to join their team
Our facility has a 20 bed CARF accredited Brain Injury unit.


Must be a Licensed PT or OT to apply

QUALIFICATIONS/GENERAL REQUIREMENTS:


  • Bachelors or higher Degree in a Rehabilitation specialty
  • Experience in working with neurologically impaired patients in inpatient and/or outpatient settings.
  • Physical Therapist, Occupational Therapist or Speech Therapist

JOB FUNCTIONS/DUTIES AND RESPONSIBILITIES:


Assist the Administrative Manager of Neurological Services

  • Implementation and revision of Program policies and procedures.
  • Implementation of in service and continuing education programs for staff, residents and/or family members.
  • Development and implementation of an interdisciplinary care planning process to address patients' treatment needs including discharge and followup plan.
  • Serve as liaison between patients, families and staff to ensure that patient and family
- concerns are addressed.

  • Ensure program compliance by patients and staff.
  • Development and implementation of ongoing QAPI activities for the Neuro Program.

Specific Duties:


  • Maintain and update roster of all admissions to the program.
  • Send memo/alert to all staff regarding new admissions.
  • Maintain and update a rehabilitation schedule for all patients on program and share with all staff.
  • Ensure initial Physiatry consult is requested.
  • Ensure initial Psychology/Neuropsychology/neurology/optometry consult is requested.
  • Schedule care plan meetings and alert staff as to who is being discussed at a particular meeting.
  • Create a folder for each patient being discussed with associated forms in shared drive for all disciplines to complete before the meeting.
  • Ensure that all meeting associated forms are completed accurately and in a timely fashion.
  • Schedule family meetings and family education day.
  • Provide psychoeducation to patients and families.
  • Provide rehab treatment to neuro patients as necessary for optimal function of program and department.
  • Conduct cognitive remediation groups.
  • Conduct staff education activities.
  • Support team with insurance appeals process.
  • Participate in the screening/admissions process.
  • Provide information regarding the program to outside agencies and families/friends of prospective patients.
  • Provide tours of the facility to outside agencies, family members and other interested parties.

Salary is commensurate with experience
