JOB DESCRIPTION SUMMARY


The Weekend After Hours Registered Nurse plans, organizes, and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.

The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.


ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES


The Weekend After Hours RN works with a team of LVN's and is primarily responsible for the nursing evaluation and admission of assigned patients in consultation with DPCS and Medical Director as well managing symptoms of high acuity patients.

All members of the Cabrillo Hospice clinical teams ensure appropriate communication and plan of care coordination to all internal/external customers.


A detailed list of responsibilities is below:

  • Executes initial written plan of care for hospice patient.
  • Collaborates with IDT and Medical Director as appropriate.
  • Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice.
  • Carries out the Cabrillo belief that relationships go far beyond the minimum care required.
  • Maintains timely patient documentation using "point of care" electronic charting.
  • Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
  • Provides health care instruction to patient and family members.
  • Orients, instructs, and trains assigned personnel.
  • Performs admission assessments.

Primary Service Locations:
San Diego County and Riverside County


Schedule:
After Hours - Friday, Saturday, and Sunday


Qualifications

  • Current CA RN license.
  • At least 1 year of experience as a professional nurse within the last 3 years.
  • Current CA driver's license with proof of insurance.
  • Computer skills [MS Word and Electronic Medical Record (EMR) system]
  • Excellent observational, written, and verbal communication skills. Understands hospice philosophy and issues of death/dying.
  • CPR certification required.
