- Provide services to patients including but not limited to ambulatory care, taking health histories, performing assessments, and conducting health education and counseling.
- perform assessment/data collection in an ongoing and systematic manner, focus on physiologic, psychological, and cognitive status.
- Develop plans of care based on patient assessment ensuring appropriate follow-up in accordance with established nurse practitioner protocols.
- Triage and refer patients for further care based on severity of illness and symptoms.
- Efficiently and effectively review labs, radiology reports and consults and communicate results and form management plans.
- Help in educating patients 1:1 or small group workshops / videos on common health conditions and concerns.
- Prepare timely, legible, and complete documentation of all patient care as provided by law, regulation, and established policy.
- Apply safety principles as identified by established policy.
- Ensure compliance with legal issues including but not limited to patient confidentiality and risk management; ensure compliance with AAAASF, JCAHO, federal, state, and local regulations.
- Able to work collaboratively with multiple health professionals.
