Job Description:


Scan-Harbor Family Renewal Center is seeking a dynamic Social Worker to provide coreservices within a Prevention/Substance Abuse Treatment Program in the South Bronx, New York.

Social Workers/Case Planners assist caretakers in meeting their children's needs, prevent abuse/maltreatment, help to enhance parenting skills, and assist families with other concrete services needs.


Responsibilities:


  • Conduct comprehensive assessments to identify clients' needs, strengths, and goals.
  • Develop individualized care plans in collaboration with clients, their families, and other professionals.
  • Provide ongoing counseling, support, and guidance to clients in a compassionate and nonjudgmental manner.
  • Coordinate and facilitate access to various social services, including housing, healthcare, education, employment, and mental health resources.
  • Advocate for clients' rights and wellbeing, ensuring they receive appropriate services and support.
  • Maintain accurate and uptodate client records and documentation in compliance with organizational and legal requirements.
  • Collaborate with a multidisciplinary team of professionals, including therapists, healthcare providers, educators, and community organizations.
  • Attend case conferences, meetings, and training sessions to enhance professional knowledge and skills.
  • Stay informed about relevant policies, regulations, and best practices in social work.

Requirements:


  • Bachelor's or Master's degree in Social Work or a related field.
  • Proven experience as a Social Worker or Case Planner, preferably in a social services setting.
  • Strong knowledge of social work principles, theories, and practices.
  • Excellent assessment and counseling skills.
  • Ability to develop and implement effective care plans based on client needs.
  • Knowledge of community resources and the ability to connect clients with appropriate services.
  • Empathy, compassion, and the ability to build rapport with diverse populations.
  • Strong communication, interpersonal, and problemsolving skills.
  • Ability to work collaboratively in a team environment.
  • Proficient computer skills for documentation and case management purposes.
  • Valid driver's license and reliable transportation (if required).
  • Provide case management and advocacy services. In addition, experience working with children and families
  • Strong organizational skills, high attention to details, effective communication skills, computer proficiency and knowledge of ACS Prevention Services and case planning
  • Connections and PROMIS are a plus
