Job Summary:


Responsibilities:


  • Conduct comprehensive assessments of individuals and families within the community to identify health needs, risks, and resources.
  • Develop and implement individualized care plans in collaboration with clients, incorporating preventive measures, health promotion strategies, and community resources.
  • Provide skilled nursing care, including health screenings, immunizations, wound care, and medication management, to individuals and families in various community settings.
  • Deliver health education and counseling to individuals and groups on topics such as nutrition, exercise, chronic disease management, and healthy lifestyle behaviors.
  • Collaborate with community partners, including schools, churches, social service agencies, and healthcare organizations, to coordinate and implement health promotion activities and outreach events.
  • Participate in community needs assessments, program planning, and evaluation to identify gaps in services and develop strategies to address community health priorities.
  • Document all client interactions, assessments, interventions, and outcomes accurately and in compliance with regulatory requirements.
  • Advocate for clients' rights and access to healthcare services, and provide referrals to appropriate resources as needed.
  • Participate in interdisciplinary team meetings, case conferences, and community coalitions to promote collaboration and coordination of care.

Qualifications:


  • Valid Registered Nurse (RN) license in the state of practice.
  • Minimum of two years of clinical nursing experience, with experience in community health, public health, or ambulatory care preferred.
  • BLS certification required; additional certifications such as ACLS and PALS may be required depending on clinical setting.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work effectively with diverse populations and in various community settings.
  • Commitment to promoting health equity, social justice, and culturally competent care.
