Your Responsibilities:

Maintain current and accurate information on all acute hemodialysis treatments performed.
Maintain dialysis equipment as per protocol.
Charting, Administer medications, monitor patient vital signs, and provide pre

- and post-procedure care with expertise as per hospital policy and procedure.

Document post treatment hospital assessment form for each patient dialyzed in the acute setting.
Provide personalized patient care, including assessment, treatment planning, and education on disease management, nutrition, and exercise.
Educate patients and caregivers on home dialysis machine operation, ensuring understanding and confidence in selfcare.
