Job Overview:
Responsible for planning, managing, implementing, and evaluating the operations of the Department of Case Management which includes oversight of effective resources utilization and the provision of high quality and excellent care.
Qualifications:
- Required_
- NJ Clinical Social Work License (LCSW)
- Three years of clinical experience
Scheduling Requirements:
- Day position
- Full-Time
- 40 hours per week
Essential Functions:
- Supervises Social Work operations of the inpatient medical and surgical units.
- Provides assistance to the Assistant Director in supervisory functions of the Case Management Department.
- Provides support and education to social work staff, including oversight of CEU programs and training. Offers clinical supervision as needed.
- Discusses illness trajectory and advanced directives with chronicallyill and/or medically complex patients.
- Identifies patients appropriate for palliative and/or hospice care and discusses these services/benefits with patient and family. Makes referrals as needed.
- Exhibits strong knowledge of advanced directives (ie POLST) and laws around healthcare decisionmaking.
- Identifies the need for interdisciplinary or family meetings and facilitates these meetings.
- Collaborates with department leadership to streamline processes and implement strategies to create more efficient delivery of care.
- Ensures compliance with regulatory documentation standards.
- Understands the financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication. Collaborates closely with physician advisor.
- Participates in interdisciplinary rounds and facilitates communication with the interdisciplinary care team.
- Completes psychosocial assessments and implement patientcentered care plans that meet needs identified in assessments.
- Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements, hospice, community resources, and transportation arrangements.
- Participates in quality data collection and risk management referrals
- Collaborates with department leadership regarding ongoing performance management of frontline social workers.
- Ensures that staff operate within compliance with all applicable policies, laws and regulations.
